Placement Forms

 

 

 

Filling out the placement forms will help you determine where you want to be placed, how you will serve, & what you hope to take away from the experience.

placed in an En/Route course where I was told I would be doing community service…Part of me was looking forward to serving others, while another part of me worried that it would further complicate my busy schedule with homework, basketball, and a social life. As it turned out, En/Route was a valuable and meaningful experience, and I’m very glad I did it.

Don't Forget the Forms! 

Throughout this year, there will be several forms that need to be filled out either by you, your supervisor, or both. Below each form, there are is a description of the form as well as directions about what to do with them. Please read these carefully to ensure the form is saved with your information entacted and can be printed or emailed to the appropriate person.  

 

*All forms must be saved to your computer prior to filling them out or the information you add will be lost. 

Pre-Town Meeting Worksheet 

Due by August 21st

The Pre-Town Meeting Worksheet will help you discern which community partners you are interested in serving. Please complete it before our 1st Year Orientation on Fri. 8/21 or email it to your professor (Dr. Gosselin, Dr. Taylor, or Dr. Vartabedian) or even print it out to bring it with you. 

  1. Save form to your computer.

  2. Open saved form & enter the requested information.

  3. Re-save the form & either email or print it. 

Post-Town Meeting Reflection 

Due by August 28th

The Post-Town Meeting Reflection will help you continue the discernment process by ranking and explication your top placement choices. Please complete it and either email it to your professor  (Dr. Gosselin, Dr. Taylor, or Dr. Vartabedian) or print it out to bring it with you to class by Fri. 8/28. 

  1. Save form to your computer.

  2. Open saved form & enter the requested information.

  3. Re-save the form & either email or print it. 

Personal Safety & Property Release 

Due by September 21st 

The Personal Safety & Property Release is a standard form all all Service-Learning students must fill out prior to beginning work at their placement. Please sign it, save it, and email it to the Center for Service-Learning.  

  1. Save form to your computer.

  2. Open saved form & enter requested information.

  3. You must digitally sign form before re-saving it. 

  4. Email form to Center for Service-Learning

Learning-Work Agreement 

Due by September 22nd 

The Learning-Work Agreement is completed collaboratively between students and their placement supervisors. While students & supervisors fill it out together, the supervisor will need to digitally sign it, save it, and email it to the Center for Service-Learning.  

  1. Supervisor, please save form to your computer & open saved form with student.

  2. Students & supervisors enter the requested information together.

  3. Supervisors must digitally sign form before re-saving it. 

  4. Email form to Center for Service-Learning

Midterm Hourly Tracking Form

Due by October 9th

The Midterm Hourly Tracking Form is first your service timesheet. Please complete the top of the form, print it & bring it with you every time you work at your placement as well as on your community partner tours. At midterm, you must bring this completed form to the Center for Service-Learning, Main Hall 214.  

  1. Save form to your computer.

  2. Open saved form & enter the requested information at top.

  3. Re-save the form & print it. 

  4. Bring form with you to placement tours & have tour guide sign it. 

  5. Bring form with you eveytime you work at your placement.

  6. At the end of each workday, fill out what service your performed & how long you served before having it signed by your supervisor. 

  7. During midterm week, please bring this hardcopy to the Center for Service-Learning in Main Hall 214. 

Final Hourly Tracking Form 

Due by December 11th 

The Final Hourly Tracking Form is your second service timesheet. Please complete the top of the form, print it & bring it with you every time you work at your placement. At the end of the semester, you must bring this completed form to the Center for Service-Learning, Main Hall 214.  

  1. Save form to your computer.

  2. Open saved form & enter the requested information at top.

  3. Re-save the form & print it. 

  4. Bring form with you eveytime you work at your placement.

  5. At the end of each workday, fill out what service your performed & how long you served before having it signed by your supervisor. 

  6. During final week, please bring this hardcopy to the Center for Service-Learning in Main Hall 214. 

Learning-Work Evaluation 

For Students

At the end of the semester, both students & supervisors must complete a Learning-Work Evaluation. In this document, students will give an overall evaluation of their performance as well as respond to their supervisor's evaluation. Then students must digitally sign it, save it, and email it to the Center for Service-Learning.

  3. Students must digitally sign form before re-saving it.

Learning-Work Evaluation 

For Supervisors

The Learning-Work Evaluation is completed by the placement supervisors at the end of the semester. In this document, supervisor will give their overall evaluation of the students' performance, digitally sign it, save it, and email it to the Center for Service-Learning.

For Students

Any student that is interested in touring a Denver Public School placement (Cole, Skinner, or North) needs to download this application. Then fill it out and send it to your faculty member as soon as possible.
